Modifier 95: Synchronous telehealth via real-time audio and video
Modifier 95 identifies a service delivered by real-time, interactive audio-video telecommunication. It works together with the place of service code, and the audio-only companion modifier is a separate designation with different coverage.
What it covers
The modifier tells the payer how the service was delivered; the place of service tells it where the patient was. Both must be right, and mismatches between them are a high-volume, entirely preventable denial category.
Audio-only delivery uses a different modifier and is covered far less consistently. Coverage rules in this area have changed repeatedly and differ between traditional Medicare, Medicare Advantage, Medicaid, and commercial plans.

Denial & audit triggers
Modifier and place of service mismatch
POS 02 with the patient at home, or POS 10 with the patient elsewhere, denies as a coding inconsistency.
Code not telehealth-eligible for that payer
Eligible-code lists differ and change. A code payable by telehealth under one plan may not be under another.
Cross-state licensure
The patient's location generally governs licensure. Provider enrollment that does not cover that state produces denials that look like modifier errors.
